Register as a Client

To register as a new client, please complete this form*.  If you are unable to do this, you can contact our equine practice on 01638 663150 and register your details with a member of our office team.

Please note that ALL COMPULSORY DATA FIELDS, denoted with an asterisk*, MUST BE COMPLETED in order for the form to be submitted.

If you are an existing client and would like to register a new horse, pony or donkey with us please call us on the above number.  Please note that if a horse of yours has been registered with us previously (either for first opinion or as referral to our hospital), your details may already be registered with us, so please call the practice to check.
